Mental Health Support Services

Mental Health Skill-building Services (MHSS) is an intensive, home-based service that is designed to assist the client in obtaining independence or stability within the community. MHSS is available for adults who have long-term mental health needs, cognitive deficiencies or dual diagnoses.

Qualifications:

-History of psychiatric hospitalization

-Crisis treatment in a residential or facility setting

-Recent release from incarceration, formerly under the care of a psychiatrist


Community Stabilization

Important Note: Crisis Stabilization (H2019) transitioning to Community Stabilization (S9482).  Community Stabilization services are short-term and designed to support an individual and their natural support system following contact with an initial crisis response service or as a diversion to a higher level of care. Providers deliver community stabilization services in an individual’s natural environment and provide referral and linkage to other community-based services at the appropriate level of care. Interventions may include, brief therapeutic and skill building interventions, engagement of natural supports, interventions to integrate natural supports in the de-escalation and stabilization of the crisis, and coordination of follow-up services.

Substance Abuse

ASAM 2.5 Partial Hospitalization Program

  • 20 hours of structured, skilled therapeutic services per week.
  • Includes individual counseling, group therapy, psychoeducation, and case management
  • Access to medication management
  • Withdrawal Management Services
  • Health Literacy counseling and psychoeducational activities
  • Medical, psychological, psychiatric, laboratory, and toxicology services, which are available by consult or referral
  • Emergency services available 24 hours a day and seven days a week.
  • Close coordination through referrals to higher and lower levels of care and supportive housing
  • Designed for individuals with acute addiction symptoms or co-occurring disorders who need more structure than intensive outpatient(IOP) but do not require 24-hour nursing.
  • Services are provided by licensed professionals and certified substance abuse counselors(CSACs).
  • Occupational and recreational therapy

When is 2.5 SAPHP appropriate?

  • Transitioning from residential/inpatient treatment (step-down care)
  • When standard IOP is insufficient to prevent relapse
  • Need for daily structure due to risks of homelessness, severe social isolation, or difficulty with daily living.

Group Home

One Path One Purpose has a residential group home in Chesterfield Virginia serving individuals with developmental disabilities. Around the clock supervision is provided by caring, professional, and trained Direct Support Professionals (DSP) to support individuals. We provide the following services:

  • Medication management
  • Developing independent living skills
  • Employment services, doctor’s appointments, community activities
  • Assistance with activities of daily living including meal preparation, housekeeping, and personal hygiene.
